Grounded Path Support provides Independent Support Coordination within the NDIS space, with a focus on life transitions, capacity-building, and steady partnership.
My work is centred around supporting school-aged participants, young people moving into adulthood, and individuals navigating key decision-making periods such as employment exploration and supported accommodation pathways.
This is not crisis-driven coordination.
It is proactive, thoughtful, and grounded in long-term development.
Being grounded means providing calm, consistent support during uncertain times. It means thoughtful planning rather than reactive decisions.
There is no one-size-fits-all approach. Each participant has different goals, strengths, and priorities.
Support is not about taking over.
It is about working alongside participants and families with care.
Support that is steady, transparent, and tailored to each individual journey.
I prioritise open and straightforward communication with participants, families, and professionals. This includes clear explanations, regular updates, and collaborative conversations so everyone feels informed and confident in the direction being taken. There is no pressure — only shared understanding and thoughtful planning.
Transitions and decision-making require careful structure. I assist with mapping out next steps, identifying appropriate services, and ensuring supports are aligned with long-term goals. Each stage is approached with intention — reducing overwhelm and increasing clarity.
Steady coordination builds trust. I aim to provide reliable follow-through, realistic timelines, and practical guidance that participants and families can depend on. Support should feel stable — not reactive or uncertain.
